Workers' Compensation Program GuidelinesThe most valuable resource that the Denver Public Schools has to perform our mission is our skilled and trained work force. Any time one of our employees is injured on the job, DPS will take an active and deliberate role in the resolution of all claims. The goal of the workers' compensation program is to get every injured worker possible back to work as soon as possible after the injury. The DPS workers' compensation program is dedicated to providing information and assistance to aid the recovery of our employees.

DPS REPORTING PROCEDURE IF THE INJURY OCCURS DURING THE NORMAL WORK DAY 2.Secretary will have employee fill out a Notification of Injury form [Word file] and issue an Authorization for Treatment form [Word file]. 3.Secretary will fax the completed Notification of Injury form to the Workers' Compensation Office at (720) 423-3695 as soon as possible or no later than within 48 hours. 4.Secretary will retain a copy of this report at the school/department. 5.Employee will report to one of the DPS Occupational Medicine Clinics for treatment. (A list of these facilities and locations may be found on the back side of the Authorization for Treatment form.) The Authorization for Treatment form must be presented to the health care provider at time of treatment. 6.If the injury is life or limb threatening, call 911 and go an emergency medical center. If possible, use Rose Medical Center Emergency Room. 7.Should the seriousness of the injury require the employee to be transported to a facility other than HealthOne, the employee's supervisor must be notified immediately. ANY SERIOUS OR UNUSUAL ACCIDENT SHOULD BE REPORTED IMMEDIATELY TO THE WORKERS' COMPENSATION OFFICE 24 HOUR PHONE LINE AT (720) 423-3281. 1.If the situation is an emergency, call 911 and go to an emergency medical center. If possible use Rose Medical Center Emergency Room. 2.Employee must notify secretary/supervisor about injury as soon as possible. If necessary, initial medical treatment should be obtained at one of the DPS Occupational Medicine Clinics. Use the Rose Medical Center Emergency Room only if the injury is life or limb-threatening or if the injury occurs after clinic hours (6 pm - 7 am M-F, Saturday and Sunday. 3.Secretary will have employee complete a Notification of Injury form. 4.Secretary will fax the completed Notification of Injury form to the Workers' Compensation Office at (720) 423-3695, as soon as possible or within two working days of the notification. A copy of this report should be kept at the school/department. NON-EMERGENCIES 1.If the situation is not an emergency, the employee should notify the secretary during the normal work day. If necessary, initial medical treatment should be obtained at one of the designated DPS Occupational Medicine Clinics before the Notification of Injury form is prepared. 2.Secretary will have employee complete a Notification of Injury form and issue an Authorization for Treatment form. 3.Secretary will fax the Notification of Injury form to the Workers' Compensation Office at (720) 423-3695 within 48 hours of the notification. A copy of this report should be kept at the school/department. Modified Duty When an employee is injured on the job and unable to perform the essential function of his or her position, the employee's supervisor or designee will assign him or her to a position where any temporary job restrictions can be accommodated. Accommodating an employee's job restrictions will help control costs associated with lost time on the job. When a physician approves an employee's return to modified duty with job restrictions and the school or department finds it difficult or impossible to assign the employee to meaningful tasks within his or her own school or department, the employee is sent home, with pay. Unless the restrictions can be accommodated, the employee is allowed to stay home and receive full wages for four months. After the four-month period has elapsed, the employee is placed on Temporary Total Disability (TTD) and will receive two-thirds of salary up to a maximum amount. Employees will, in most cases, recover more quickly and return to work sooner if they are allowed to work within their job restrictions. The Modified Duty Program was created with this purpose in mind. Under the Modified Duty Program, a work pool has been established for employees who have been injured on the job and have been released by their treating physicians with restrictions that prevent them from performing a necessary or essential function of their positions. Employees placed in this work pool can report to any school or department on a modified duty assignment. If employees are assigned to work at a school or department while on modified duty, the original department or school will pay their salary. Closed Medical System Under provisions of the Workers' Compensation Closed Medical System, all covered work-related injuries must be treated at one of the DPS Occupational Medicine Clinics or Rose Medical Center Emergency Room. Should you seek medical treatment for a job-related injury from your private physician or through another clinic or hospital, you will be financially responsible for that care. If the injury is a crisis situation requiring emergency care, a life- or limb-threatening injury, you can seek treatment at the nearest health facility. PROCEDURE FOR USING THE CLOSED MEDICAL SYSTEM 1.At the time of the injury, you should notify your supervisor and report to the nearest clinic. 2.Your supervisor will initiate an Authorization for Treatment form to be signed by the principal or supervisor at the location at which the injury occurred. 3.You are required to complete a Notification of Injury form which your school/department secretary will fax to the Workers' Compensation Office. 4.You may call the nearest HealthOne/OccMed clinic for an appointment. A map listing the locations of the clinics are on the Authorization for Treatment form. 5.You must present the Authorization for Treatment form at one of the designated DPS Occupational Medicine Clinics. 6.Call 911 for LIFE- OR LIMB-THREATENING job-related injuries. 7.A written Work Status form will be given to employees at each clinic visit. The employee is to return the Work Status form to your supervisor. 8.If your medical treatment requires physical therapy, the first week of physical therapy appointments will be scheduled by the HealthOne clinic staff. Subsequent appointments may be scheduled by you and should be arranged to not conflict with your work schedule. Restrictions Many injuries are minor and the injured employee can return to work without any restrictions. This however isn't always the case. The treating physician may established any set of restrictions that he/she feels will speed the recovery of the injured employees. These restriction must be followed by both the injured employee and their supervisor. If the restrictions cannot be met at the primary job site then the modified duty program is the first alternative. (See above) Missed work Student Interns (DPS Students Out in the Workplace) DPS carries the workers' compensation insurance coverage on all students we place into internship programs or job shadowing programs. This requirement assumes that the students are not on any payroll. If an employer hires a student, and the student is receiving credit for the work experience, the gaining employer becomes responsible for the workers compensation coverage. DPS does not want to assume this liability. This liability belongs to the gaining employer as the party making the payroll. General liability coverage for students in an intern program or job shadow program would follow the same general guidelines as workers' compensation. If the student is not receiving any wages they will be automatically covered by the DPS policy. If they are receiving a wage, then DPS may be able to cover them. However, this must be coordinated each time with Risk Management as our insurance policy only automatically covers students in intern programs who are not being compensated. Student Teachers
