Reporting Property LossesBreak-Ins, Vandalism, Property Damage, Theft Click here to download a Property Loss or Damage Report form. NOTIFY: Safety & Security: Call (720) 423-3911 to report all break-ins, vandalism, and damaged or stolen property. Police Department - Report all break-ins, thefts and vandalism by going online: Denver Police Department Online Offense Reporting System. Or call (720) 913-2000 for offenses that require a police officer to respond. To obtain a Case Report Number (at least 72 hours after a report is filed), call (720) 913-6029. Facility Management - During normal working hours, call (720) 423-4020 to report facility property damage directly to Facility Management. After hours, call Safety and Security Dispatch at (720) 423-3911. Fixed Assets Department - For all DPS fixed assets (barcoded items), complete an Equipment Activity Report (EAR) and send it via school mail or fax (720) 423-4109 to the Fixed Assets Department/Service Building. Keep a copy for your records. RISK MANAGEMENT - Complete a Property Loss or Damage Report form, include the Case Report Number (see Police Department above), and send via school mail or e-mail yolanda_encinias@dpsk12.org in Risk Management. NOTE: If property is on loan, the borrowing school/department is responsible for the reporting procedure. For questions, call Risk Management at (720) 423-3613.
