Reporting Employee Injuries

• Report all employee injuries by completing a Notification of Injury form [Word] available from your school/department secretary.  Fax the completed form to the Workers' Compensation Office at (720) 423-3695.
  
  
• If an employee needs medical treatment, get an Authorization for Treatment form [Word] from the school office, department or the Workers' Compensation Office.  Seek medical attention at any one of the six DPS Occupational Medicine Clinics. If an injury is life or limb-threatening or for after hours emergency care, go to Rose Medical Center Emergency Room.
