Off Campus Trips

While the majority of field trips are less than one day, some trips are overnight, extended excursions, or high risk activities.  These Off Campus trips must be approved by the building principal and Executive Director of Educational Services.  Submit the application below at least three (3) weeks before a planned Off Campus Trip. Use the Parent/Guardian Release Agreement and Durable Power of Attorney forms below.

Accident Insurance for Off Campus Trips

When off campus trips are approved by Joe Sandoval, Executive Director of Educational Services, and information is forwarded to Risk Management, an additional accident insurance plan covers these extended excursions.

When a student injury occurs on a field trip or extended excursion, a Pupil Accident Report form must be completed and forwarded to the Risk Management Department.
