Student Accident Insurance

DPS insurance does not cover student injuries due to accidents at school or during school-sponsored activities, such as field trips. At the beginning of each school year, the District provides parents with information on a voluntary accident insurance program. For students who are not covered by a family health or accident insurance plan, parents should consider the student accident insurance available through the District. Student Accident Insurance:
  • Is available to parents of all DPS students.
  • Provides low-cost accident insurance for students.
  • Is available on a full year 24-hour basis, for school time only, for football coverage and extended dental coverage.

Applications for Student Accident Insurance are distributed at registration. If you have any questions or need to file a claim, contact Summit America Insurance Services at (866) 313-4512, or visit their website at www.k12studentinsurance.com.  To download a claim form, click here.

Report all student accidents by completing a Pupil Accident Report form and sending it to Risk Management.  Pupil Accident Report forms may be downloaded here.  They are also available in the school office or may be ordered from the Warehouse in packages of 25 (Stock No. 01-164012).
