Reporting School/Site Incidents (Safety and Security)

The following incidents shall be reported in writing on the School/Site Incident Report form and submitted by Fax or email to DPS Safety and Security within 24 hours of the incident:

  • Injuries to students and staff
  • All weapon related incidents
  • All assaults by students or others on another student or staff member
  • All incidents where students are arrested on school property
  • Malicious damages to District property (fire, graffiti, etc.)
  • Losses of district property
  • All sexually related incidents
  • Any incident that an administrator wants documented for future reference
