Reporting Visitor Injuries

Report all visitor injuries using the Pupil Accident Report form. Cross off the word "Pupil" at the top of the form and write in "Visitor". Complete the form and send it to Risk Management via school mail or fax. The forms may be downloaded here or are available in the school office. The forms may be ordered from the Warehouse in packages of 25 (Stock No. 01-164012).

FREQUENTLY ASKED QUESTIONS

Q:  When should I report a visitor accident or injury?

A:  When a visitor is injured on school grounds, complete a Pupil Accident Report form (cross out "Pupil" and write in "Visitor") and forward it to Risk Management via school mail or fax within three (3) working days.

Q:  Where do I get a Pupil Accident Report form?

A:  Click here to download a Pupil Accident Report form.  The forms are also available in your school office or may be ordered from the Warehouse in packages of 25 (Stock No. 01-164012).

Q:  Who should complete the Pupil Accident Report form?

A:  The employee with the most firsthand knowledge of the incident should complete the Pupil Accident Report.

Q:  Where do I send the completed Pupil Accident Report?

A:  Send the completed Pupil Accident Report to Risk Management via school mail or fax.
